Your First Visit - Patient Forms
Thank you for trusting your vision to Grossnickle Eye Center! If you are seeing one of our doctors for the first time or if your personal or insurance information has changed, you will need to fill out some paperwork. For your convenience, you may download the online patient forms, fill them out, and bring them to your appointment.
If you would prefer to complete these forms when you arrive for your visit, we ask that you arrive about 15 minutes earlier than your appointment time.
Tips for a Successful Visit
We look forward to seeing you soon! To ensure your visit is as convenient as possible, we recommend that you:
- Please leave enough time in your schedule to accommodate your appointment. The typical eye exam takes approximately 45 minutes to an hour to complete. We try to see every patient at the appointment time, but please understand that dilation and other procedures such as visual field testing, photography procedures, and cataract evaluations require more time than routine appointments.
- Notify our staff at least 24 hours prior to your appointment time if you are unable to make it. This allows us to ensure appointments are available to others.
